General Rules of the PRISM Lab
- Please help us to keep the PRISM Lab tidy at all times!!
- Please clean up your workbench after each use (clean all scraps and garbage and anything that you are not working on).
- There will be NO eating or drinking tolerated in the PRISM Lab (bottled water only please).

1. Borrowing
1.1 Equipment
- We do NOT permit the borrowing of any equipment from the lab for any reason.
1.2 Tools
- We do NOT permit the borrowing of any tools from the lab for any reason.
- Please return tools to the Tool Cabinet when you are not using them.
1.3 Books
- Please use the sign up sheet on top of the bookshelf in the lab to sign in/out books.
1.4 Laptops
- If you need to borrow a laptop, please contact Catherine Carter (x3209).
- All laptops will be kept on top of the black storage cabinet in the PRISM Lab.
2. Ordering: Supplies, Equipment
Please read this carefully. If you are unsure about anything please contact Catherine Carter (x3209).
2.1 Overview
- If you are unsure of where to purchase items, please contact your supervisor for suggestions.
- Please refrain from ordering items that cost >$100.00 unless approved verbally by your Supervisor (Graduate Student or Post-Doc) and Catherine Carter or Tom Chau.
- All orders >$1000 MUST be put on a Purchase Order (PO)
- If you purchase items with your own money, please hand in all receipts to Catherine Carter room 4W 375. All receipts MUST have the following information (or you will not be reimbursed):
- Vendor Name
- Item Name(s)
- Total Cost
- If you require assistance processing your order please send Catherine a link or email with the following information:
- Product number/model
- Product description
- Quoted price
- Vendor/company.
2.2 Items <$100.00:
- You can purchase these items yourself, please send receipts to Catherine Carter for reimbursement.
- If you would like to order online, please send Catherine Carter or your student supervisor a quotation including the following:
- Vendor Name and Address
- Contact name and phone number
- Item(s) to be ordered with the ordering codes (if applicable)
- Total Cost and currency
2.3 Items $100.00-$1000.00 (after taxes and shipping)
- You MUST obtain approval from either your supervisor or Catherine Carter (please notify Catherine of ALL requests and approvals).
- You can order this online (same guidelines as above for <$100)
2.4 International (including US)
- Please ask your supplier to use our customs broker, who is Thompson Ahern. Phone number: 905–677–3471. Otherwise:
- There will be unnecessary delay getting through customs and,
- We will have to pay a lot more money!
- If you have questions please contact our materials manager, Doug Creelman (x3440).
3. Reference Letters
If you require a reference letter from Tom at any time, please DO NOT EMAIL TOM DIRECTLY. You are required to email Catherine Carter (ccarter@bloorview.ca) instead. Catherine will make sure there is no delay. She will take care of everything much faster than if you were to send directly to Tom.
For all requests you must include the following:
- The type of Reference needed (online, fill out forms etc..)
- Please attach all relevant forms and clearly mark them (i.e. ogs, nserc, etc…) in the email attachment
- Please send the DUE Date
- If you require us to hand out a reference letter via mail, you are required to give Catherine a fully addressed envelope.

4. Ethics
PRIOR to starting your ethics process, please contact Shawna Gutfreund x3507 or room 4W 420 or via email: sgutfreund@bloorview.ca; She will help you through the process, give you helpful information and advice.
The drive ‘public-on blmredoa’ is the Bloorview Research Institute folder which contains all of the templates and instructions for the new Ethics procedures and brand new forms. Some of the forms have not been changed yet, so please use only the forms/instructions from those folders. If you don’t have access to this folder please contact Catherine Carter (x3209).
